Instructions:

To Submit your data recovery case online, you’ll need to begin by filling out the authorization and questionnaire forms.  As you complete each form, please be sure to click the “Next” button to go to the next page.  At the end, please press the submit button and the documents will be forwarded to us.

If you are planning to drop off locally, please note the following changes due to COVID-19:

Lobby is closed to customers. Local customers may submit cases using our drop-box. Please fill out the case submission forms before drop off.
