I believed this is suitable for Head swap.
when tried to transfer adaptive from donor to patient using MRT, getting
error "[highlight=yellow]SFW not found[/highlight]"

It's rarely necessary to transfer head adaptives, and given that MRT can't even accurately read the head map out of the patient, I wouldn't trust it to work anyway.

What happens when you just use the original patient ROM on the drive with the donor heads installed? Terminal log?

With original ROM giving some error in reading sysfiles (28,35,301 etc)

can write via serial port at low baud rate but can't read even successfully written sysfiles.
so no user area access.

Most likely either the donor heads were damaged or the drive's SA is just too badly damaged to be repaired. Have you retested the heads back in the donor to see if they are still good?
